New Patient Registration

At the Animal Clinic, we are excited to welcome new patients to our family. To help us get started, we ask that you fill out our new patient registration form. This form will provide us with the information we need to ensure that your pet receives the best possible care, and will help us to get to know you and your pet better.

Payment Options

We understand that the cost of veterinary care can sometimes be unexpected. That's why we offer a variety of payment options, including financing through CareCredit. By offering these options, we hope to make it easier for you to provide the best possible care for your pet, regardless of your financial situation.

Cancellation Policy

We understand that sometimes, unexpected things come up and you may need to cancel your appointment. However, we ask that you provide us with at least 24 hours' notice if you need to cancel your appointment. If you cancel your appointment less than 24 hours before it is scheduled to take place, you will be subject to a penalty/cancellation fee charge of $75.
